Results: This study focused on the association of MC4R gene variants with body mass index (BMI) and obesity-related complications, and how these variants affect body weight and metabolic health by influencing signaling pathways. The MC4R gene was found to contain a large number of low-frequency variants. The majority of MC4R variants result in loss of function, but some still result in gain of function. MC4R variants are not only associated with BMI but also with risk for a range of metabolic diseases. Overall, MC4R gene variants provide new perspectives and potential targets for the treatment of obesity and related metabolic diseases.

What are the effects of changes in the MC4R gene function?
Among the variants in the MC4R gene, loss-of-function variants are associated with high BMI, higher incidence of obesity, severe obesity, type 2 diabetes, and coronary artery disease. In contrast, gain-of-function variants are associated with low BMI, lower incidence of obesity, lower diastolic blood pressure, and lower resting heart rate.

What is the MC4R gene?
MC4R, a member of the G protein-coupled receptors, is a peptide secreted in the medial hypothalamic nucleus. It regulates body weight, energy homeostasis, and feed intake by binding to melanocortin. The MC4R gene responds to energy metabolic signals such as leptin, hunger hormone, and neuropeptide Y (NPY) to stimulate pro-opiomelanocortin (POMC) and agonist agouti-related peptide (AgRP) neurons in the hypothalamus, which in turn regulate energy metabolism and feeding behavior in the body.
