Summary: In this study, researchers projected the 10-year fiscal impact of expanded Medicare Part D coverage for glucagon-like peptide-1 receptor agonists (GLP-1RAs) in obesity treatment using a validated Diabetes, Obesity, and Cardiovascular Disease microsimulation model. Across 30 million eligible Medicare beneficiaries, the base-case analysis estimated $65.9 billion in total drug costs, partially offset by $18.2 billion in long-term health care savings from reduced obesity-related comorbidities, yielding $47.7 billion in net increased spending. Three-way sensitivity analyses varying treatment uptake (5%-20%), adherence (20%-60%), and additional price discounts (10%-30%) demonstrated that medication costs dominate total expenditures, while improved adherence generates incremental albeit still insufficient cost offsets. These findings underscore the critical importance of rigorous budget impact analysis (BIA), cost-effectiveness modeling, and payer-centric economic evidence in informing reimbursement negotiations, pricing strategy, and market access for anti-obesity therapies.
